This Clearances Instruction Sheet is revised. It will help you make good use of the web sites to the Child Abuse, Criminal History, and the FBI Clearances below. For instance, you have the option of submitting a paper form or submitting an electronic form. Please note that the Child Abuse and FBI Clearances take a great deal of time to process (up to 12 weeks) so do not delay in submitting these clearances! |
| Act 151: PA Child Abuse History Clearance | This link will open up Pennsylvania's Department of Public Welfare site where you can download and find instructions for completing this form. |
| Act 34: Request for Criminal Record Check | This links to the Criminal History Request Form. You may download the instructions for completing this form. You may also navigate to the PATCH system where you can make this request online using a credit card. |
| FBI Clearance | This links to the Cogent Systems Fingerprint Services for Pennsylvania web site. Click on "Pennsylvania Department of Education (PDE)." Applicants must register online with Cogent Systems prior to going to the fingerprint site. |
